Useful Tips to Help You Prepare For Your Visit

Make a Note:

  • Make a note of any health concerns that you might want to talk to the doctor about.
  • Make a note of your symptoms, when they started, the order in which they appeared, and anything that makes them better or worse.

What to Bring:

  • Bring a list of any medications you take, including prescriptions, over-the-counter drugs, herbal products, and vitamins.
  • Bring a list of your allergies, especially for any medicines or foods.

Medical History:

  • Know your personal and family’s (mother, father, siblings, and grandparents) medical histories if possible, including:• Surgeries
    • Serious illnesses
    • Chronic diseases

Health Habits:

  • Think about your eating and drinking habits and how often you exercise.
  • Think about your current stress levels, how well and much you sleep, and anything else that affects other aspects of your health.

Reminders For After Your Appointment:

  • Find out if you are expected to schedule a follow-up with or contact your provider after a certain period of time.
  • Call our office if you don’t understand any of the instructions provided to you or if you have questions.

  • Be sure you understand the possible side effects of any medications your provider prescribes.
  • Tell your provider if you feel worse, have other problems, or have side effects from any medications.

Are Preventive Care visits covered?

We want to make sure you’re informed about potential out-of-pocket charges that may occur during your preventive office visit. While most preventive visits are covered at no cost to you, you may be charged for certain things, such as:

  • Discussing new symptoms or health concerns that are discovered during your visit
  • Receiving treatment or testing for an existing health condition
  • Undergoing procedures such as spirometry, EKGs, or biopsies
  • Having labs or radiology tests that are not considered preventive
